U Fun Field – Activity Park Customer Registration & Waiver Form

I, here by acknowledge that I have voluntarily chosen to participate in the activities offered by U Fun Field at the activity park located at U Fun Field.

In consideration of being allowed to participate in these activities, I agree to the following terms and conditions:

1. Assumption of Risk:
I understand and acknowledge that the activities at the park involve inherent risks, including but not limited to property damage, physical injuries, and any type of accidents. I voluntarily assume all risks associated with participating in these activities.

2. Release and Waiver:
I hereby release, waive, discharge, and covenant not to sue U Fun Field, its owners, employees, agents, and representatives (hereinafter referred to as the “Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, serious injury or any type of accidents that may be sustained by me or any property belonging to me, whether caused by the negligence of the Released Parties or otherwise, while participating in the activities at the park.

3. Indemnification:
I agree to indemnify and hold harmless the Released Parties from any and all liabilities, costs, expenses, claims, demands, actions, or causes of action, including attorney fees, arising out of or related to my participation in the activities at the park.

4. Insurance Coverage:
In the event of any injury or illness, I authorize the activity park staff to administer first aid and, if necessary, seek emergency medical treatment for me. I understand that U Fun Field maintains insurance coverage for participants. However, if any medical costs exceed the coverage provided by the park’s insurance, I acknowledge that I will be personally responsible for such additional expenses.

5. Personal Belongings:
I acknowledge that I am solely responsible for the security and safety of my personal belongings while at the activity park. I release the Released Parties from any liability for loss, theft, or damage to my personal belongings.

6. Photography and Videography:
I grant permission to U Fun Field to use any photographs or videos taken of me during my participation in the activities at the park for promotional purposes without compensation.

I have read and understood this form, and I am aware that by signing it, I am waiving certain legal rights. I voluntarily agree to be bound by its terms and conditions.
